Players are grouped by age and/or ability. The clinic reserves the right to rearrange groups and/or time based upon enrollment. Participants must wear USA Hockey approved safety equipment. All participants will receive camp jerseys.

Medical

The Arrows Hockey Clinic and St. Sebastian’s School do not provide medical insurance. In the event of illness or injury requiring treatment, hospitalization, and/or surgery, family medical insurance must be used.

Waiver

I understand that participation involves some risk and that the Arrows Hockey Clinic and St. Sebastian’s School, or anyone else associated with the Hockey Clinic or St. Sebastian’s will not assume any responsibility for accidents resulting in medical or other expenses.

Further, I give permission to the School and Hockey Clinic to use any photograph taken for future promotions.
