Checklist for parents

»	The applicant should take either the Independent School Entrance Exam (ISEE) or the Secondary School Admissions TEST (SSAT) and designate St. Sebastian's as a recipient of the official score card. The ISEE School Code for St. Sebastian's is 221545. The SSAT School Code for St. Sebastian's is 6934.
»	The parents or guardians of the applicant should complete the Parent Questionnaire in the Application for Admission.
»	The applicant should independently complete the Applicant Questionnaire (Parts One and Two) of the Application for Admission. Part One should be submitted prior to (or on the day of) the applicant's personal interview.
»	The application should be signed and submitted to the Admissions Office with a nonrefundable fee of forty dollars($40). The deadline for submission of all application materials is January 15.
»	A personal interview is required of each applicant. Please call the Admissions Office to schedule an appointment. Interviews are conducted during the academic day only.
»	The applicant should write his name, current grade, and current school on each of the four required recommendation forms before submitting them to his recommenders. Recommendations should be sent directly to St. Sebastian's by the recommenders in the self-addressed envelopes provided in the application packet. Recommendations should NOT be submitted to teachers for completion prior to December 1.
»	Parents who have requested financial aid consideration should call the Admissions Office to request an Application for Financial Aid and the Instructions on Applying for Financial Aid at St. Sebastian's. The deadline for submission of financial aid materials is February 1.
